address, phone number, e-mail address, etc., please

box next to each updated information line. When
finished, click the Submit Data button.
Your updated information will be sent to the
office. You should then receive a confirmation
that your information has been received. If
you do not receive a confirmation, please contact
the manager and give her the information.
