* 12 MONTH TERM AGREEMENT
- When signing a "12 month term agreement", Customers will be
committed to maintaining DSL service for 12 months.
If customer fails to complete the 12 month term, they will be responsible
for the remaining monthly charges of the 12 month term.
All remaining charges will be due in full at the time that services are
terminated.
NOTE: We do offer two monthly billing options for
your convenience. You may choose to have your DSL charges added to your
phone bill or you have the option of having your DSL charges billed on
a separate billing statement. Monthly charges are due by the 16th of each
month and are subject to late fees after the 16th. If payment is still
not received by the 27th of the month, services will be subject to being
interrupted until full payment, including all additional fees, is received.

* Once your request is received,
you will be notified of the total amount due for all applicable charges
including taxes, as well as when your service agreement will be ready
for you to sign. The final request for service will not be processed until
payment is received and the service agreement is signed and completed.
